Service we offer to all colorectal cancer patients

• Information re treatment options
• Emotional support
• Help applying for grants and benefits
• Symptom Control
• Discharge planning
• Liaison with the community Macmillan services
• Liaisons with other appropriate health care professionals e.g.Stoma care team, pouch nurse, chemotherapy team
• Support Group
• Advice re local support and counseling services

Referral Criteria

• We will see all patients with a known colorectal cancer diagnosis at any stage of their cancer pathway.
• Wherever possible we would like to meet with patients at the point of diagnosis
• Inpatients can be referred by telephone or by bleep.

Hours of Service

We offer a Monday- Friday service 08.30-4.30 (at present this is Mon-Fri until 2pm)

Multi Disciplinary Team Meetings and Ward Rounds

There is a weekly MDT every Tuesday 12.30-13.30. All patients with a diagnosis of colorectal cancer need to be discussed at the MDT. All details to be given to Cancer Pathway Coordinator xt. 4118.
